For Returning Parents

We are  happy that you will be returning to Pingree in September.

In the coming months, there will be much information that we want to share with you and forms we will need to receive from you. Please check back as we add to this page.

Please note that the dress code has changed for the 2010-2011 academic year.  You can review the current handbook for changes.

In the meantime, please click on the links below for forms that must be returned to Pingree's Director of Health Services Elizabeth Robitaille, R.N., B.S.N., M.P.H. These forms must be returned to her by August 15, 2010 at Pingree School, 537 Highland Street, South Hamilton, MA 01982. For your privacy, they need to be downloaded and sent by regular mail. They can not be filled out online.

School policy requires that each student have an annual physical exam, including updated immunizations.

The Emergency Information form is the source of any students/guardian information should there be a medical emergency. This information should be kept current. Please report any changes to the school as soon as possible, in order to ensure your child’s safety. The over-the-counter medication form lists the medications that are available at Pingree. By state regulation, over-the-counter medications can be administered only with parental consent. STUDENTS SHOULD NOT CARRY ANY TYPE OF MEDICATION when on campus, EXCEPT PRESCRIBED INHALERS AND/OR EPI-PENS.

Additionally, you will find a Precription Medication Order form for prescribed medications. Should your child be on a scheduled medication(s) that need to be taken during the school day, (including inhalers or an epi-pen), please have physician complete the order form below.
